Please enter your LAST name only (properly capitalized) and your ASA membership ID number without any leading zeroes. If you have a space or hyphen in your name, type your name as if they did not exist (e.g. Le Baron = LeBaron). If you have a "Jr." or other addition to your last name, omit it (e.g. Doe, Jr. = Doe). Remember, correct capitalization is important and omit any leading zeroes in your ID number.
